Submit a Film

1:. Download (right-click and save target as)

2:. Open the file as Read Only and complete ALL fields.

3:. Save a copy of the completed form and email as an attachment to main@filmi.org with subject: FILMI9 FILM SUBMISSION.

4:. Print a hard-copy (turn off form field shading) of your signed and completed form and mail to the address provided at the end of the form.

All completed forms that are received will be confirmed via email. Incomplete forms will not be accepted or returned.
